2023/2024 School Year

The 2023/2024 school year is right around the corner! Below is the required paperwork for next year. This year, we're going paperless! All forms can be accessed and filled out virtually. Once the form is completed you can save the file by clicking the save icon in the top right corner. After the form is saved you can email the paperwork to Miss Livas at jlivas@stphiliplutheran.com. If you would prefer to turn in the forms directly, you may do so at the front office. Make sure to read the instructions carefully as some paperwork is not required for all students. 

All paperwork must be completed by the first day of school August 22nd.
